Nursing Coach — Separate Electronic Records Notice
1. Records covered
We ask separately for your consent to provide Nursing Coach account agreements, purchase disclosures and receipts, notices of material changes, and related legally required consumer records electronically. This consent is separate from accepting the Terms of Use or authorizing the one-time purchase. It does not authorize marketing, microphone access, patient-data processing or future charges.
2. Access and retaining copies
Electronic records are delivered as readable HTML pages and service email, with download/print options. You need an email account you can access, a supported modern web browser, an internet connection and a device that can display, save or print HTML and email. A printer is optional if you can save the record. Before consenting, open the displayed notice and agreement, verify you can read them and use the offered save/print control. The separate affirmative choice confirms that access. If a changed delivery format creates a material risk that you cannot access records, we give the required revised requirements and opportunity to withdraw.
3. Paper and contact information
You may request a paper copy of a covered record without charge by emailing john@appalachianintegrity.com or calling (540) 524-2290, identifying the record and a suitable delivery address. Do not include patient records, passwords or full card details. Tell the same contact if you need to update the account email or cannot receive records; an available account-email control may also be used. We confirm identity reasonably. A public residential address is not required from you merely to read electronic records.
4. Withdrawal and consequences
You may withdraw electronic-record consent through the same email or phone contact. We explain any effect on features that require electronic delivery and arrange any paper records required by law. Withdrawal does not itself delete your profile, authorize another payment, refund a purchase or erase a valid paid entitlement. We do not charge a withdrawal fee. We will not rely on withdrawn consent to provide future legally required written records solely through an inaccessible electronic route. Existing delivered records and legally supported evidence may be retained under the Privacy Notice.
5. Separate affirmative choice
Your separate unticked choice states: I consent to the electronic records described in this notice. I opened the notice and can read, save or print these records. I understand I can request paper copies or withdraw without a fee using the stated contact. We record the applicable notice version and your separate affirmative choice. This electronic-record consent does not replace acceptance of the Terms of Use or authorization of a payment.
